My tires were replaced at the Lexus dealer 5000 miles ago with a set of Bridgestone Potenza RE 040 215/45 R17. One tire has since developed thread separation. Has anyone else had problems with this tire?

What happened when you went to the bridgestone store to have the tire inspected?
5000 mi is very low for separation not caused by road hazard.
